If the array is running code Release 29.*.012 or later or 30.*.512 or later, the logs should be reviewed to determine if there are any disk errors, as under some circumstances, disk errors can trigger the 995 LCC error, so if a review of the logs shows that one or more disks on the same bus are exhibiting large numbers of errors, then replacing the disk(s) first is the best course of action.   Continue to monitor the logs moving forward, and if further 995 errors occur then this message is an indicator that the reporting LCC or attached cable has exceeded its error counts and should be replaced. Because it is difficult to determine whether the initiator is the cable or the LCC, it is recommended that both be replaced.

If the array is running releases earlier than 29.*.012 or 30.*.512, the message could be benign. The appropriate action for earlier releases would be to look for other indicators of a back-end problem (media errors, general bus or disk issues, etc.)  And if nothing is found, the incident could be cancelled.

In addition to the above recommendations, you should upgrade to Release Release 29.012 or later or 30.512 or later to take advantage of updated algorithms that do a better job at identifying failing drives and using proactive copy to migrate data to more stable drives.

FLARE Release 04.29.000.5.012 or later and 04.30.000.5.512 or later makes this a legitimate dial-home that indicates problems with the LCC, cable, or disks on that bus that must be addressed.
