Having trouble creating a new initiator

Let's see what silliness I am forgetting to do now...  In a nutshell, I am going through the "Create Initiator Record" dialog and I have what I believe to be correct entries in place:

Initiator Name: VMware 1
SP-Port: A-4
Initiator Type: Clariion Open
ArrayCommPath: checked
Unit Serial Number: Array
Failover mode: 1
Vendor: QLogic
Model:QLA2462
New Host: checked
Hostname:uussth01vhcp011
IP address: 172.24.4.101

With all this data set, the OK button is grayed out. What am I doing wrong here?  This is on a CX3-40c

Reason for manually creating: pre-seeding the entry on the SAN

WWP/WWN - no place in that dialog to put this information - that I can see

I guess I misread the help screen for that window.

Initiator Name  User-defined name for the HBA initiator

Init name is wrong and that's why the OK is grayed out. WWNN:WWPN of the HBA

Initiator name should be port wwn:nodewwn

Is there a reason you are creating it manually ? Did you specify node:port wwn
