SCv3020 MS SQL Best Practice

HW INFO
2x Dell PE R640, CPU 2x Xeon 2.2GHz, RAM 256GB, 2x 16GB SDCard, 2x 10GbE, 2x 1GbE, 2x 8Gb FC
1x Dell SCv3020, 10x HD SAS 1.8TB 10k 2.5", 4x 16GB FC
1x Dell SCv300, 12x4TB NLSAS 3.5" 7.2k
2x Brocade 300, 8x 8GB FC
 
SW INFO:
-ESXi 6.5 with vCenter and vMotion
 
I am being asked to configure storage and present the volumes to ESXi
The volumes will be used for SQL server
 
QUESTIONS:
1. Because it has Tier2 and Tier3 HD in dfferent box.
How can I create volume for vmware.
Shoud I create 1 volume for each Tier box or create 1 big volume which span from Tier2 to Tier3 disks
Why I asking that because Tier2 and Tier3 HD has different speed. I never done this before.
Usually I have the same disk spec.
 
2. How big the block size for SQL.
Should I choose 512kb or 2Mb (default) or 4Mb
 
3. For SQL data and log
Should I 
a. create separate disk in ESXi, 1 for SQL data and 1 for log
b. create 1 disk in ESXi for both SQL data and log
