Why after buying and extended warranty, my wife cant get help from dell support.

Onsite personal one on one software support is available but it is VERY expensive.

Call Customer Care with the order number of your warranty extension to have them make sure it was properly applied to your account/system.  Note the length of your warranty extension ... if you bought a system 3 years ago and purchased a 1 year extension of your warranty, that would cover the system for two years, meaning it is out of warranty now.  (This is a hypothetical example, of course, since you didn't mention what system you have or when it was purchased.)
