U2713H USB3 issue, any formal exchange procedure yet?

I've purchased the U2713H back in Jan. As many other early revision user the card reader and the usb port is faulty. The card reader is not reading properly. and one of the usb port on the side of the monitor wouldn't do anything beside charging my cell phone when plug in. 

As one other thread, some dell rep has responded, people with this issue should get a new replacement? So I am just wondering if there is a formal procedure out there yet to initiate the exchange. Thank you.

All of the replacements going out now should be the fixed revision, A02. Since your monitor has a Service Tag number, contact Dell Technical Support for your country. Provide to them the following data so they can setup a monitor exchange -

Shipping Address:
Phone Number:
Dell (27"/30") Service Tag number:
Monitor 20 digit alpha-numeric PPID number:
Troubleshooting Performed:
Reason: U2713H A01 has USB port and Card Reader issues
