PowerEdge 2650 "Missing Member"

I have a PowerEgde 2650 with 5 drives in a RAID 5.  One of the drives crashed with the blinking amber light so I ordered a replacement through Dell.  The PERC controller sees the drive but under the array configuration utility in the BIOS it still lists it as "missing member". 

I have tried booting the server with the drive pulled out.  After the RAID BIOS loads I tried inserting the drive.  The drive went green but the array never rebuilt.  A few times I got it to say "rebuilding" in the BIOS but after a while (less than 15 mins) the drive was back to flashing amber.

The server is running Fedora.  This is what it displays in dmesg:

I installed afaapps-4.1-0.i386.rpm but when I run afacli this is what I get:

Does anyone have any tips?  I'm about to wipe the array and start over but this doesn't build my confidence for the future.
