PowerEdge 2950; Failing drive 1:0:4, replaced (hot swap) and new drive generates "Unexpected sense" errors
Power Edge 2950, a 5 disk RAID 5 array on a PERC 6/i Integrated controller. 2 Virtual disks.
all disks are Seagate ST3250620NS; Firmware 3BKS
In July I had 1:0:4 showing errors, amber blinking light.
I acquired the same disk, however, Firmware 3BKH.
I hot swapped, rebuild completed. Within a day I started seeing "Unexpected Sense" errors.
System functioning, but showing status as 'Degraded'.
Power failures in building forced a shutdown, and reboot. In log I see:
"The current firmware version 6.2.0-0013 is older than the required firmware
version 6.3.3-0002 for a controller of model 0x1F0C: Controller 0 (PERC 6/i
Integrated)"
1. How do I upgrade this firmware, and where do I get the upgrade from?
Secondly, after reboot completed I see a series of errors in the Alert Log (Dell OpenManage)
Unexpected sense. SCSI sense data: Sense key: 3 Sense code: 11 Sense qualifier:
0: Physical Disk 1:0:4 Controller 0, Connector 1
Controller log file entry: Physical Disk 1:0:4 Controller 0, Connector 1
escalating to:
The Patrol Read has started.: Controller 0 (PERC 6/i Integrated)
Patrol Read found an uncorrectable media error.: Physical Disk 1:0:4 Controller
0, Connector 1
A bad disk block could not be reassigned during a write operation.: Physical
Disk 1:0:4 Controller 0, Connector 1
Device failed: Physical Disk 1:0:4 Controller 0, Connector 1
So I removed this 'new' disk, and hot swapped back the original that was failing. It worked for about a week, and then started showing errors again.
On the recommendation of an IT acquaintance, I saved the Alert Log, then cleared it.
Then hot swapped the replacement (new) drive back in again.
Now I get no more Alert log events, and the disk 1:0:4 drive light blinks Amber, Green, then off, repeatedly.
What can I do? I can't tell if the drive is rebuilding, or doing anything?
Any help anyone can give is greatly appreciated. Due to cutbacks our entire IT dept was let go, and I'm stuck looking after these servers. I suspect other drives will be failing soon too (Murphy's Law).

Depends on your OS. Assuming Windows, you can run the Update Package(s) for Microsoft Windows from the OS to update the firmware. You can get the packages from support.dell.com. Update the BIOS first, then ESM/BMC, then the PERC (driver first). I would also update the drive firmware so they are all at the latest version.
Sounds like your replacement drive was faulty, so you got errors and the drive failed. Reinserting the original faulty drive gives errors too, because it is faulty (you knew this). I would look for your money back on the replacement you purchased - test it with diagnostics to be sure. Amber/green alternating means "predictive failure" and should not be used.
I would suggest getting a new replacement for disk 4, rebuild, then update the system firmware.

Question: Would the Firmware of 3BKH on the replacement have any negative impact on the fact that all of the other drives have firmware 3BKS?
Or should the drive have just worked fine?
by "Test it with diagnostics to be sure" - is there something on the system (win2k8R2) that I can use to test the disk? or do I have to reboot the server to the controller and use some diags on the PERC 6/I controller?
Considering I have almost 2TB of data on here, backup might be an issue. I am attempting to copy all data off onto a drive share I have a couple TB available on. This is a VM Host machine.

If the controller's firmware was really out of date compared to the newer drive firmware, it could have caused some strange log entries, but there is seldom enough difference for it to cause the drive to fail.
You can download Dell's Online Diagnostics and run it to test the drives from the OS, or you can download and boot to their 32-bit Diagnostics to test the drive outside of the OS.

Thanks for this initial advice. With the replacement drive installed (3BKH), I've downloaded, installed and am executing the Dell Online Diagnostics. Everything checked out on the initialization, running self tests now on that drive. If it's ok, I'll post results here and look for further input, thanks.
