PowerEdge 2950-PERC batteries not charging

You can get false battery errors with the controller being out of date. I would suggest updating them before doing anything else. Have you also tried reseating the batteries as well?

Well, we updated the firmware on the PERC5 and PERC6, everything was OK. Now we are seeing battery failures again. The batteries will go to ID 2048 error code and the battery fails. After rebooting the server, they will come back up to ID 2358 indicating that they are charged. If you go into Open Manage Server Admin, click on battery properties, it still indicates battery has failed. Any ideas?

Replaced batteries twice, then battery/cache DIMM and PERC 6e together as a set, then entire chassis and only transfered the HDD's.

Have you already reseated the battery? What version of Open Manage are you using as well? 

Where are you seeing these errors? Have you cleared the hardware log previously?

Error log was cleared on the original chassis 3 weeks ago.
Error is only showing in Open Manage at this time.  Was seeing battery
error while booting on original chassis.
