Hi,
without more information it is not possible to give a definitive answer if the hard drive should be exchanged or not.
But as there are a couple of medium errors it could be that the hard drive is dead and should be replaced.
You may try to update the firmware of the drive and see if it will solve the issue, but I cannot guarantee it.
