device reported an error on a request to read data from media

Have a less than one-year old PowerEdge 1600SC with VS80 tape drive for 5 nights-a-week backup. The "clean media" LED has illuminated only four times in 10 months. The fourth time was early last week. Cleaned the drive with the cleaning tape and it worked for one night. Since then, using three different tapes (all the same age, less than a year old) we have gotten three consecutive failures at verify. The error message reads "

Bad data. There may be a hardware or media problem." The MMC Event Viewer states, " The device, \Device\Tape0, has a bad block."

 

Does anyone know what is going on with this issue? Thank you in advance for your help.

What type of cleaning tape are you using? Is it light brown in colour ?
