Thanks for the image. Did you have latest iDRAC and PERC FW installed on the server. Can you also check whether iDRAC reporting any issue on this storage controller

The picture you uploaded is not showing properly. If you have service tag on the picture it will not get uploaded as it is sensitive information. Can you upload the image after hiding the service tag. Can you also share the server model and error message shown. If you have access to iDRAC, can you check controller status on iDRAC also.
