Just to add to Dmitry's comment. When you reboot the RPA press Ctrl+R and re-initialize the virtual disk 0.

There is no official procedure to the best of my knowledge.
If you truly need to completely reset it, you would need to boot into BIOS, format the hard drives and reinstall from a DVD.

Reboot the new RPA and press Crtl+R (you'll see the prompt for Ctrl+R). Re-initialize the virtual disk 0 and insert the 3.4.2 ISO. Reboot the RPA. Wait until the OS install, log-in as boxmgmt and assign a temproary IP. Launch deployment manager to add the new RPA to the existing cluster.

It might be an access issue. You might not have access to it but I don't know why you wouldn't. Try this direct link:
https://powerlink.emc.com/nsepn/webapps/btg548664833igtcuup4826/km/appmanager/km/secureDesktop?_nfpb=true&_pageLabel=servicesDownloadsTemplatePg&internalId=0b014066801a418b&_irrt=true
If that still doesn't work I recommend you open a ticket with EMC Support and ask them to give you access to RP ISOs.

How about a Gen5 box?
On our Gen5 boxes there is no Ctrl-R option. At boot it says Ctrl-G to get to the raid controller bios, but when I do so the system appears to stop with just a cursor on a black screen where it sits.

OK, try the following.
During the boot process use Ctrl+G to enter the BIOS. Disable Port 60/64 Emulation and reboot. Enter the BIOS again and then initialize the virtual drive 0 and re-enable Port 60/64 Emulation. Then reboot once more.

How about if I were to boot from a CentOS DVD to format the drive, then re-install?
Would that remove some critical piece?
