ISCSI setup on windows server 2012

Can anyone provide any information about how to setup ISCSI on windows server 2012 so i have connectivity and how to make it shared to all of my hyper-v servers?

i have 6 network ports dedicated to storage,  3 on each iscsi network.  10.33.16.X and 10.33.17.X.  when working with the iscsi initiator how do i know which ip to select on the source(server) side,  do i  need to set a target for all 3 or is that where MPIO comes in to play?

what roles does the unisphere host agent come into play is that required?

how do i make these data volumes shared across multiple hyper-v servers?
