Symptoms
End device FC port directly attached to an MDS Fabric switch go to errored state after ISSU/ISSD performed on the switch.
This has been noted on departmental level switches (non-director class) directly connected to any GeoSynchrony code level on VPLEX VS6 only.
This issue is due to the open FCNS PLOGI connections not being restored correctly on the MDS switch after ISSU/ISSD. As a result, when the locally attached end device next queries the FCNS, instead of sending a reply the MDS send an FCNS PLOGI request to the end device. The end device may not handle this second connection request well and go to an unknown or errored state.

Upgrade to NX-OS 8.4(2d) or higher.
Workaround:

The issue is addressed by either:

Manual reload of the switch AFTER the upgrade has been completed, OR
-
Shut the VPLEX ports associated to switch PRIOR to the upgrade, and bring up after upgrade has been completed
