PowerPath: PowerPath Management Appliance common issues

Summary: The purpose of this KB is to list basic items to check when troubleshooting PowerPath Management Appliance (PPMA) prior to opening a Service Request (SR) with support.

Cause
There are many things that can cause issues with PPMA. Below is a list of some of the most common issues that can be checked prior to opening an SR with support.

Basic checks
Version - Ensure that the PPMA version is still supported on the ESM.

Compatibility - Ensure that PPMA and vSphere versions are compatible per the ESM.

License - If this is a redeployment the hostname and IP must be the same as in the license. 
  Served licenses are tied to the hostname/IP. 
  Unserved licenses are tied to the UUID (must be re-hosted every time ESXi is updated on the PowerPath host)
