What update software are you trying to use? Also, what is the OS being used as well? If you haven't tried it yet, this is the specific 7.03.06.00 update. Also, what OpenManage products are you using, and what are their versions?

What update software are you trying to use?
A: used to install SUU 14.12.200.69 for auto detect driver required to update.
Also, what is the OS being used as well?
A: default WIn2008 R2 comes with Dell
If you haven't tried it yet, this is the specific 7.03.06.00 update.
A: tried 7.03.06.00 update as what you mentioned. failed. (it simply say 'This update package is not compartiable with your system configuration')
Also, what OpenManage products are you using, and what are their versions?
A: Dell OpenManage System Management Software (64-bit) ver 8.2.0

What you can try is to download and extract the H200A_A09_ZOE,exe from here .After extracting files then
1. Copy extracted files to bootable media.
2. Reboot system to bootable media.
3. Execute the batch file called flash.bat
4. After flash is complete, remove bootable media.
5. Reboot system for changes to take effect.
If the update doesn't run correctly then you can try running with the /forceit command after the filename.

We had exactly the same problem with firmware 00.00.00.00 on two controllers.
We could finally resolve the issue by deleting and reinstalling the firmware:
Download the LSI management software megarec contained in this package:
http://www.mediafire.com/download/ukhbhrqj4sh6lzq/LSI-9211-8i.zip
Wipe the old firmware using these commands:
megarec -writesbr 0 sbrempty.bin
megarec -cleanflash 0
Reboot the machine and write the new firmware from the dell package:
sas2flsh -o -f h200a.fw
This did the job for us and now both controllers are working as expected.
Now, the card has no BIOS extension, so you will not be able to hit CTRL-C to configure the card. The configuration needs to be done from inside the OS.
If you want the BISO extension back, execute:
sas2flsh -o -b mptsas2.rom
