I purchased a Dell 4700 with the 128MB Raedon X300SE video card. The output includes

1 DVI terminal

1 VGA  terminal

1

The Radeon X300SE does not include a 59 pin connector as shown in the Radeon X300 guide . The connectors are:
 
1    VGA output connector
1    DVI -I output connector
1    S-Video output connector
 
As I recall only one VGA cable was included and no DVI to VGA adapter.
 
Maybe the link in my previous post will be helpful. I cannot answer your other questions on dual monitors.Good luck
