dell ptiplex 780 losing operating system

Hi, i have dell optiplex780 with the following specification. Q9400, core 2 quad @2.66ghz, ram=4 GB, hdd= 650. The pc always losing the operating system.My question is why the pc is losing the operating system suddenly. The pc after formatting will work fine but afte a few days it will lose the operating system.

If the drive is bad perhaps it should be replaced. Not sure what you mean by "Losing"
