DW5802 Mobile Broadband on Windows 8.1 (Dell Latitude E6430)

I am unable to get Dell Wireless DW5802 LTE/WWAN Mobile Broadband module to work on windows 8.1.  

Out of the box, Windows 8.1 installs drivers for DW5802.  After the fresh install, and very first time, the module works.  I get the network connectivity and browse the web.  However, once you shutdown the computer and reboot, the module stops working after that.  Is anyone able to get DW5802 module working on windows 8.1?

I have tried installing all new and old drivers from Dell's support page for e6430, upgraded the firmware, verify BIOS and windows 8.1 network settings.  I have even contacted Dell support.  However, this remain an issue.

Current Firmware: 4.08
