I don't have anything documented as HBAs fail very seldom these days or maybe i am just lucky but this is what i would do:

1) Bring system down and replace HBA.

2) Go into Cisco CLI/Fabric Manager, remove old WWN and add new WWN to zones ( actual steps will vary based on how you do your zoning, device name vs fcalias vs port zoning). Active zoneset

3) If connected to Symmetrix you can use this handy command to replace old WWN masking with new WWN

symmask -sid 123 -wwn old_wwn replace new_wwn

4) If connected to Clariion open Navisphere Manager and verify you're in the Storage Tab. Expand the array and then the Hosts section. Right-click the host that had bad HBA and click Connectivity Status. You will see the initiator (new HBA) that need to be added as they will show up in ~management. Click Reconnect to have them added to the Storage Group.

5) Now on the host go into Disk Management and hit rescan. Bring up command line and run powermt restore and then powermt display. If you do not see correct number of paths to each device you might have to reboot the system.
