Port Failure on N2000

Two of our N2048 switches have had random port failures, by failure I mean while the connect led and activity led are lit, we are not able to get a DHCP address.  A reboot (reload) will "fix" the issue/port.  This happened on 5 ports and two different switches.  The remaining ports worked normally. 

Afterwards, we upgraded the firmware form 6.5.1.5 to 6.5.3.4.  

Has anyone seen this?  Here is one of the ports that failed:

Hi,

What are the ports connected to? Are there any errors in the logs? What is acting as the DHCP server?

Two were just standard Dell Workstations and two were Ubiquiti access points.  Our DHCP Server is a Windows 2016 Server.  All other ports seemed to be operating normally.  I did not review the logs.
