The contents of event such as "\Device\AFA0 : ID(0:01:0); Error Event [command:0x28]" or "\Device\AFA0 : ID(0:01:0); Unit Attention [k:0x6,c:0x29,q:0x2]". Hi Jimmy,
The error messages are indicating a possible problem with the hard drive at Scsi ID 1. The first step in troubleshooting this problem is to upgrade the drivers and firmware for the Perc 3/di in your PowerEdge 4400.
PERC2, 2/Si, 3/Si, 3/Di, Driver, 2000, v. 2.7.1.4944, A12
Filename: w2ka12dr.exe [211 KB]

Adaptec PERC3/Di, Firmware, PowerEdge 4400, v. 2.7.1.3571, A10

I would also recommend running the Elite Diagnostics on this hard drive
Dell Elite Hard Drive Diagnostics, MS-DOS, 5.30.00
