T310 Degraded Virtual Disk, all Physical Disks "online"

This is a PowerEdge T310 server. Physical disk 0:0:3 has shown physical medium errors in the past, but after reseating the drive everything was fine for awhile. Now Dell OM is reporting that the virtual disk is degraded, but all four disks are reporting online and have that green check mark by them.

Nothing is showing predicted failure and I don't see any more disk medium error's, All firmware is up to date--is their something I'm not seeing as to why my virtual disk would fail? I'm thinking their is still an issue with PD 0:0:3 even if it's not showing up in the logs. Any input would be largely appreciated. Thank you.

Sometimes the PD status under the Perc S300 controller may not be that accurate. And since all your firmware is up to date, swap out the problematic PD 0:0:3 with another PD and let us know the status of your Virtual Disk.
