The IO operation at logical block address retired

Good Day Each day I get the following errors: The IO operation at logical block address 0x40028 for Disk 3 was retried. I get three of the same errors with three different addresses. Listed under event 153 In Disk manager I have Disk 0, Disk 1 and Disk 2 and CD-Rom 0. So what is disk3? Disk 0 is OS etc (2 x SAS - green LED on) Disk 1 is user data ( 4 x SAS - Green LED on) Disk 2 is a USB drive The server is a PowerEdge T320 Server 2012 Standard I tried to install Dell Support Assistant. I keep getting "an unexpected error occurred while running this request. I followed their pointers, updated browsers both firefox and chrome, enabled what they recommended in the browsers, tried disabling Anti-Virus etc but still crashes, tried reinstalling it too. Would it even help me to diagnose my disc issue? Thank for any assistance.

Hello

It sounds like an operating system error. Disk 3 is likely a partition and not a physical or virtual disk. If you install OpenManage Server Administrator it may provide additional information that could help you troubleshoot the issue. You can find the OMSA download on the system support page.
