Hard Drive not detected in OS on Precision rack 7910

My main OS drive is a 500GB SSD and have 4-1TB HDD's under RAID-0 configuration from a vendor. The OS drive started acting since few days and now stopped booting into OS. When I try to repair the OS using the reinstall CD, the OS is unable to detect the drives but the BIOS shows all 5 drives. 

Under the "PERC H330 Mini BIOS Config Utility - VD Mgmt", I can see that all five drives are on the same RAID (00:01) with the SSD being 00:01:00 and the rest are 00:01:01 to 04. The SSD is listed as unconfigured physical disk. Any help in getting me repairing the OS or re-installing is greatly appreciated.
