Dell PV-100T DDS4 Tape wont eject

Hi I have a PowerEdge 1600SC Server with a Dell PV-100T DDS4 Tape Backup. My backup tape will not eject out of the drive and the 3 front LED lights are blinking. I have tried restarting while holding down on the eject button but this failed to solve the problem. I have been a few days now without backups. Getting a little worried.

Could this be a case where the drive has gone dead?

Actually I finally got this to work. I had to shut down the server and actually un-plug it. Waited a couple seconds, plugged it back in and held the eject button down as I restarted. It came out within the first few seconds of the boot up.

I have (had) the same problem at a customer site.  turned out that cancelling the still-running veritas 9.0 job and then pressing eject got the tape out.  tape is a year and half old, so we are swapping it out for a new one, will clean the drive (light is lit, but not flashing) and look for a firmware and driver updates.  Let me know what you found out...
