Unisphere LDAP Connection Fails with Missing bindDn

I'm trying to set up an LDAP connection to Active Directory. I filled in all of the fields in the Server tab (except for certificates). When I hit apply I get an error that reads "Failed to establish connection with xxx.xx.xx.xxxx: Missing bindDn and password". I get this error even though the bindDn and password clearly there. I'm using the same AD settings that I used for my XTremIO, so I know they work.

What could be causing this error?
