Is the user trying to login using an Avamar local account (setup within Avamar) or using an LDAP-integrated directory service?  If the latter, what format is being used when entering the user name?  Are all of the other parameters correct (reminder: the "Domain" field refers to the Avamar domain you want to log in to, not the directory service "domain" in which your domain user account may reside).
