Hello JG,
I would recommend calling in to support and opening a case to work on this. I do not feel we can assist you in a timely manner in this forum.

Follow up FYI for anyone else in my situation: DSITV 4.1.2 web plug-in for vSphere 6.5 Web Client
Support Engineer indicated that this is a known issue and that it will be fixed in a new release that will be released end of June (this month).

Although I am sorry you hit this issue I am glad my colleagues were able to get an answer for you on this issue.
