Its best getting the switch logs for review and opening a case with the Emc Connectivity team when you have the above type of issue. As troubleshooting steps can vary based on the outputs in the switch logs.

Hi,
With the info provided, I would reseat module 1 and see if the issue reoccurs. If not, probably replace it.

I listed two common articles below which have troubleshooting steps that can be tried.
Cisco Switch: Module remains in powered down state after SanOS upgrade

Cisco MDS Switches:
Module Powered-dn Unable to power up card (Power_OK not set)

Yet what is usually tried is powering off and on the module to see if it it comes back online in the correct state.
i.e
show module '2'
poweroff mod '2'
no poweroff mod '2'
Then wait a minute and issue a 'show module 2' again to see if its back online in the correct state.
Usually if the module doesn't then come online after being reset it probably needs replacement.
