BAD_POOL_HEADER
I have a dimension 4400 to which I added a USP2 controller card. I recently added a Dell monitor with USB hub.
I have no problems with anything hooked to the main system but when I try to use the hub I get a blue screen and BAD_POOL_HEADER as the result.
I've only tried it a couple of times with a 256 Mb USB2 memory card. In both cases it complete copying a couple files to the card, then went to the blue screen.
I have all the latest updates on the system.

The monitor hub is connected to the Added USB2 PCI card. Anything connected to that seems to work. It's just when I connect to the Dell hub the problem occurs.
I've found what I think are the latest drivers and installed them.
I also just found that it can go to a blue screen a few minutes after plugging anything into the hub. No activity is required.

What does Dell way about the built in USB? Does it require special drivers? If you plug it into one of the original USB ports does it do the same thing?
Do you have the BIOS at the latest version of your 4400?

Where are you connecting the USB hub in the Monitor? Are you connecting it to one of the original USB ports on the PC or to a port in the PCI card?
If I'm correct, the built in USB ports on a 4400 are 1.1. The monitor is probably 2.0, although 2.0 is supposed to be backward compatible with 1.1. ports - just they will run at 1.1 speed.
Will the card reader work OK if it's connected directly to a port on the PC? Or will it work correctly if you connect it to a 2.0 port on the expansion card?
More info is needed to really assist you.
