I realize this reply is to an old post but it may be useful for others searching for resolution to this problem.
I believe this error is caused by a misconfiguration of AAA Authentication or the Line Console / Line SSH commands have been entered with a password for each of those. I suggest using the username you entered in your switch config and the password from either the Line SSH or Line Console commands as the password. You should then be able to access the switch.
If neither of these work, you may have to initiate the password recovery procedure to obtain access to the switch. Once you have access you can try to negate the AAA commands and/or the Line Console/Line SSH commands and try access again.
If that doesn't work, you'll likely have to reset the switch entirely (restore to factory default) and then reconfigure the switch again with extra attention paid to the Line Console / Line SSH and AAA commands (download the CLI guide for reference).
Note: If you're seeing this error on a VRTX switch and cannot gain access via any of the methods above (including the password recovery procedure) then you may have to initiate a reset of the switch by accessing the Chassis Management Controller command line interface. It it strongly advised you contact Dell PowerEdge Support for assistance resetting the switch via the CMC CLI. The command to reset a VRTX switch via the CMC CLI is racadm racresetcfg -m switch-1

cheers I figured it out. Enter the command "line console" and specify the password under that.
