Perc 3/di corrupting RAID

Hello,
 
I have a PE 2650 with a Perc 3/di integrated RAID Card.  I just added the necessary components 3 months ago that activated the onboard SCSI card (memory, battery and key.)  Both BOIS levels upto the latest version, and I installed Server 2003 using the latest OM revision and drivers.
 
My problem is I keep loosing a specific drive in the RAID 5 array.  Drive 0:1 keeps failing and going offline.  The first time this happened (a month after the initial build) I swapped another drive and set it to rebuild.  The drive rebuilt then failed again the next day.  To be sure, I moved everything off the array, completely rebuilt and scrubbed a new volume with new disks and started over.  1 day after completing the rebuild, drive 0:1 failed again. 
 
This server is out of warranty, so I don't know what I can do to troubleshoot and correct the problem.  I believe I have an issue with the controller or connectors, but don't know how to test them.

If im not misstaken you are able to view the controller logs by extracting it via OM, dont recall if it's functional on the Perc3/DI with the absolute latest OM, but you can try. Might be able to find the reason for the disk keeps failing.
 
STORAGE --> Click on the Perc3/DI --- > INFORMATION/CONFIGURATION (in the top) --> Pulldown menu (Available tasks) --> Export Log?
If this option is not available the Drivers and firmware might be out of date or the OMSS dont have the support for this function for the Perc3/DI.
 
In this log you can find what the controller is reporting about the disk, or if there is any other disk that has any problems causing the drive to fail. Also communication between controller and backplane is also viewed there as well as ROMB Memory.

Do you have the latest firmware loaded on the RAID controller?  It's listed as 2.8.1.7692 here.  On the 2650's I managed a while ago, I had better luck with them after I upgraded to 2.8.x.
 
If you have the latest, check the logs and if that doesn't help, start looking for an Adaptec or PERC PCI controller to put inside the machine.
