PERC4 Raid and Corrupted partition

Hello.  I have a perc4 on a PowerEdge 2650 setup in a RAID5 configuration with 4 drives in the array and one hot spare.  Every time I reboot, the C: drive becomes corrupted, sometimes badly enough that it can't bring up a directory.  So far, chkdsk /r has fixed it each time, but I need a little more confidence that it will not fail completely.  I will get a blue screen on a reboot.  What is wrong with this controller?  It has been flashed to the latest firmware, and the BIOS on the machine is updated to A20.

A liitle more information might help such as Service Tag, drive and PERC type.

Please supply also System ,ESM, PERC BIOS/Firmware levels.

The Service Tag number is 8C2F441; the Bios revision is A20; the PERC4/DC controller has been updated to A05.  The drives are Maxtor Atlas 15K 73G Ultra320 SCSI.   I don't know the ESM number; it looks as though the diag partition has been corrupted also.  Thanks for your response; I hope this info helps.
