R740xd BOSS "Completing a failed IOCTL request"

Hi. We have two R740xd servers with BOSS card. We use the BOSS card as storage for Windows Server OS installation.

There's no apparent problem with it, however in the eventlog, there's an error message logged every couple seconds (the eventlog is spammed by this event and it's only logged for the DELLBOSS VD):

eventlog: Microsoft-Windows-Storage-ClassPnP/Operational, event ID: 504, source: StorDiag: message: "Completing a failed IOCTL request." Details:

DeviceGUID {277f085e-6b01-55b5-70c9-69fda7212ca0}
DeviceNumber 0
Vendor NULL
Model DELLBOSS VD
FirmwareVersion MV.R00-0
SerialNumber 6ceee43e08dc0010
IrpStatus 0xc0000472
IoctlControlCode 0x2d9404

Both servers have different firmware/driver versions (one of them has the latest available):

BOSS-S1: driver 1.2.0.1048, firmware 2.5.13.3022 A06

What does this error message mean? Is it an indication of a problem or one of those 'harmless' events I should ignore?

I would consider flashing this server to have the latest drivers and firmware for the BOSS card, to rule that out as an issue. IOCTL is going to be the system making a system call to manage some IO through a driver (abbreviated explanation, there's definitely more to these calls than I'm qualified to comment on). If you're not seeing problems, I wouldn't think it would be an immediate concern, but I'd err on the side of making sure that drivers/firmware/Windows updates are caught up, to be on the safe side.

Hi Dylan, thanks for replying. One of the servers has the latest firmware and drivers, so there's not much more I can do about that. Windows is also fully up-to-date.

Same problem here, RX740XD. PERC H840, errors range from

Completing a failed upper level read request.

Completing a failed non-ReadWrite SCSI SRB request

Completing a failed IOCTL request.

Hi, you can try 2 and 3 below.

"Based on my experience, we can try the following steps to inspect the OS in first troubleshooting.

1) Set the computer to clean boot state to check for software conflicts. We can refer to this link,

https://dell.to/3uMdoLH

2)Please try the System File Checker tool to repair missing or corrupted system files to see if it helps. Detailed command, we can run “SFC /Scannow” in the CMD with admin right by referring the following docs,

3)Please check the disks mounted to the system with Cmdlets “CHKDSK”. The following discussed this commands in detail for your reference.
