Raid 5 failed after rebuild
I have a Dell PowerEdge T310 server with four hard drives configured as raid 5. The raid controller is an embedded Perc S130. I added the 4th hard drive as a hot spare but into 60% the rebuild failed. Now when I enter the OMSA it says Raid Failed. All four hard drives will flash amber at the same time and stay on for about 3 seconds and then go back to flashing green. This happens every 3 to 4 seconds. I have not rebooted or gone into the raid configuration because I'm too worried with the server not booting back up. Any idea with what is going on? Also in OMSA all the physical drives say online and ready.

Hello thanks for choosing Dell. Can we see a screenshot of the physical hard drive status from OMSA?

Hello,
that is a weird situation, the fact to have VD in fail state, but PDs are online.
I think OMSA doens't give us the real situation of the controller.
I suggest you to make a full backup, reboot and check in the controller what the situation is as first step. You can have a more clear situation.

I plan to do that this weekend, it's just very odd plus I have zero performance issues with the server.

Suggested Step here:- Upgrade the firmware of the controller(if not).
if the issue is the same there is corruption for sure.
Create a new raid but make sure you have a data backup, the last known solution from field experience.

I updated the firmware on the perc controller and rebooted. Now I can't open OMSA and the following is logged into event viewer. I did this via a remote connection and wasn't able to boot into the raid configuration but I will be on-site in the morning to do so but it looks like I got issues with two of the four hard drives. I have two spares but I believe I can only replace one at a time to try a rebuild but that will probably fail as well. Should I try to replace disk 1 first?

Replace this Disk medium error detected: Physical Disk 0:1:1 and then see what is happening, update me the latest status once you are onsite

I replaced Physical Disk 0:1:1 and was unable to boot into the OS. I put the original back in and the raid switched from failed to degraded and was able to boot back into the OS and initiated another rebuild attempt.

Hello your non-Raid’s Bus Protocol on Virtual disk doesn’t look okay. Did you previously use only 1 VD? Where did you use the new HDD added from?

please check in IDRAC if the VD is in automatic background initialization check
