can you disable one path to you storage array (to prevent corruption) and uninstall and reinstall PowerPath. I've seen some weird stuff with PowerPath on Windows .where the only way to resolve the problem for me was to remove it, bounce the box and reinstall it.
I had uninstalled and reinstalled Powerpath several times (tried version 4.5.1 and 4.6.0)...
The only combination that i hadn't tried (which ended up giving a solution last night) was to go down to a single HBA, remove powerpath and have all the luns on the connected SP. I then did the diskpar and format.
Since then (and after re-installing Powerpath), the LUNs seem to have been remembered and things are operational again.

I would highly recommend you open a case on this issue as what you described does not sound right at all.
