660f ghost drive showing in array manager

We are seeing ghost (phantom Drives) in the array manager or our system.  These phantom drives appear to be duplicates.  Ex: We have disk 1:00 which would be fine but also have another disk 1:00 which is offline.  This goes for all drives 1:00 through 1:13
 
Any help would be greatly appreciated.  We are considering resetting our configuration and rebuilding everything.

You should contact technical support.  The PV 660 COD needs to be modified to resolve this issue.

You may have a QLconfig problem - ghosting is most likely associated with seeing the drive down both paths.

what is the COD?  Is this an issue that we need to be concerned with in the future? Can it happen again? We have already rebuilt our SAN to get around the initial problem.

The COD is like the database configuration of the PV660.  It may happen again, depending on the firmware you have installed.

I have the same problem, how can I "modify the COD"?
What can I do to fixed the problem? I have been trying to call support but they let me 30 minutes on the phone w/o and answer.
