Weird disk in 660f array

After swapping in a disk to try to fix what I thought was a bad disk, I'm seeing the replaced disk as READY but also a Phantom disk "ArrayDisk 1:-1". Any idea how to fix this without recreating the array?

Did you replace the drive hot or cold? (with the system powered up or powered down)'

On a 660F you only want to replace drives when the system is powered up. The raid configuration ('database') is stored on all the drives. If you hot insert a drive any information it had gets replaced with what the array says that it should be. If you power down the array and then swap a drive, when the controllers boot up, they may see 2 different configurations between the disks, and it could cause problems as it doesn't necessarily do the 'sensible' thing and follow the majority of the drives.

Replaced it HOT, I see the disk READY with the corect ID and the Phantom with the 1:-1 thing. I can't remove the phantom and I can resync the new one.
