Did you receive any documentation from your vendor?  Most likely, they configured CHAP authentication for the iSCSI LUNs.  If CHAP was used, you'll need to find the right authentication to make that work.

Can you contact your vendor for assistance?

Thank you Karl for your reply.

I tried to workaround. i was able to connect iSCSI Initiator from Windows 2012 to EMC Server but i didn't show-up any disk from Disk Management.

I checked the Storage details from EMC server, CHAP status is not configured.

Is there any step i missing? Thanks again

Now i solved my problem. I type a wrong server name. After i changed it with the correct server name and reconfigure the iSCSI initiator, it shows all my LUN disk storage.
