Disk health code too many medium errors- Disk error

I am seeing the folllowing error message on one of my disks.

Disk health code too many medium errors

Seems to be a faulty disk. This disk also has been allocated to host production data.

How can i safely swap for a good one if its faulty?

Which storage system do you have as you didn’t state this in your post? We need this information as it helps for us to provide you the correct steps for replacement.
