iScsiPrt Errors (7/20/39/49)

Topology: Dell R620 with Broadcom 57800 quad port (RJ45) (2x1gb & 2x10gb ports) (3 machines) Windows 2012 Server Dell MD3620i (dual controller) Each machine has 2 VMs We have few Failover clusters working well on first two dell machines. We just brought up a third machine (R620) and initialized a VM (for DB node). We are UNABLE to add this node to the cluster. I have dell tickets open and Microsoft tickets open and no one can help. We properly configured iSCSI initiator and this was reviewed by dell and Microsoft support. We have MD3620i properly setup and LUNs assigned. We had the NIC card replaced to no avail. When we try and add node to cluster, we ALWAYS fail on "Validate Disk Failover" Node XYZ.com holds the SCSI PR on Test Disk 2 and brought the disk online, but failed in its attempt to read and validate file data on partition table entry 1. The group or resource is not in the correct state to perform the requested operation. In the event log of the machine we are adding, we get the errors below. Can anyone suggest a solution to this problem as we really need this node online.
