Fixes & Enhancements

.- Added support for PERC 6 controllers
.- Driver will rescan the BUS when a LD/PD is deleted, created, marked offline and/or when the controller is set to Dead to avoid IOs running to fill the system event log. PnP rescan will force the PnP Manager to send INQ to all the BUS/Target/Lun.
.- Standby resume changes to not log false FW errors.
.- Driver will scan all FW events since last shutdown when resuming from hibernation to report logical drives degraded while the system was on hibernation mode.
