If you are upgrading from firmware version 01.03.10 on model numbers OCe14xxx or OCm14xxx you must update the firmware by using the ISO image located in the Diags Kit 01.04.02,A00-00 (DPN: 5TD7N SWB,UTIL,NTWK,EMULX,MDIAGS). The Diags Kit is posted on Support.Dell.com.

Install the adapter in a system and boot from the ISO image and it will automatically update the FW.

NOTE: Once you upgraded to 01.04.xx, you cannot downgrade to 01.03.xx. Attempting to downgrade will damage the adapter.
