Symptoms
Change: Unity code was upgraded from 4.4.0.0.5.xx to 4.5.0.0.5.xx
Issue:
In Unisphere under the heading 'System View' -> 'Summary' -> 'System Health Issues', Host Initiator Warnings are appearing on Unity with OE code 4.5.
The warnings say "The initiator does not have any logged in initiator paths. Check the connection between the initiator and the storage system."
In previous Unity codes before 4.5, these warnings did not appear on the 'System View' -> 'Summary' page (the heading was "Hardware Health Issues" in codes before 4.5)
This is example of initiator alerts
"The initiator does not have any logged in initiator paths. Check the connection between the initiator and the storage system"
Setting the 'Ignore' option on the initiator, will not remove these warnings from the 'System Health Issues' section
uemcli -d -u local/joe -p xxxx /remote/initiator show -detail
21: ID = HostInitiator_56
Host = Host_12
UID = iqn.1998-01.com.vmware:mem-esx347.xxxxxxxxx
Initiator type = iscsi
Ports logged in =
Ignored = no
Health State = Degraded/Warning (10)
Health Details = "The initiator does not have any logged in initiator paths. Check the connection between the initiator and the storage system.
From Unisphere Access>Initiators Tab
From the Initiator Path tab all host initiators are showing logged in.
When clicking on each host and going to the initiator tab, they are all logged in as well.

Resolution
This issue is not caused by unity code upgrade to 4.5 code, Any issues reported would have existed at previous code level but due to Enhancement in Unity 4.5 code it will be reported in Unisphere GUI. All hosts will have access to unity storage objects Once all Hosts are logged in to unity .
These initiators are usually provided by the ESX hosts. Even if we successfully remove them from array side,on the next scheduled vCenter or ESXi host poll the array will pull then right back into the array and create the "stale" initiator records again.
These would need to be removed and cleaned up from the ESXi hosts itself.
