Symptoms
The Unity CSI node driver fails to register a Kubernetes host on the Unity array. The following errors may be present:
Note: The following log entries are only evident when debug logging is enabled in the CSI driver.
Error #1

Host initiators on the array require correction before proceeding further.]" func="github.com/dell/csi-unity/service.(*service).syncNodeInfo.func2.1()" file="/go/src/csi-unity/service/node.go:1835"
Cause
The hosts in the Kubernetes cluster were cloned from a template. Each host has the same iSCSI initiator and a misconfigured IPv6 interface.
Resolution
To resolve this issue, perform these steps:
- Ensure that the iSCSI qualified name (IQN) for each host is unique. Cloned virtual machines (VMs) may necessary to have the IQN re-generated.
- Disable the IPv6 network cards if not in use.
- Remove any stale initiators associated with the host being registered which are present on the array
Note: If the event of Kubernetes nodes booting from Unity volumes, host network addresses may already be registered to the Unity and prevent driver registration from completing successfully.
To resolve, remove any network addresses associated with the host and let the CSI driver register the network addresses automatically when the Driver Node pod is restarted.
