Aha, so even though the "Use chap credentials" is unchecked, it still tries to use it. Gnarly. I checked the box, removed the credentials, unchecked it again and saved. No more errors.

The change that fixed the error wasn't on the EQ, it was in the MPIO settings in the HIT on the VM. The VDS/VSS access was green, which I guess just means "configured" rather than "success". When it is highlighted, the Group VDS/VSS Access Settings window is displayed. Knowing what I know now, I realize that the CHAP checkbox is unrelated to the VDS settings.
