Without seeing the diags from the array and the switch logs it would be very hard to identify what happened. You should open a support case, so that the cause can be determined.
This can be caused by may things, controller fail over and the second controller isn't properly cabled, switch LACP/Lag not configured, etc.

I know this was a long time ago, but were you ever able to identify the issue with these arrays?
