In regards to the E1000 Failsafe Voltage Error, this is normally due to a damage board as you suspected, to be certain would you power down the server and remove everything but the following from the server?

1 Power Supply
CPU1 (min for troubleshooting)
System Board
Heatsink for CPU1 for prevention of over-heating
1 DIMM for CPU 1.
Control Panel
Control Panel cable
Riser Card

Try to boot under that configuration and see if there is a change, if not power down and try swapping with the other power supply.

So I have very minimal configurations, one CPU, or the other, no drive back planes, No CD, etc etc... Only a single power supply, etc. I still get the error. So it looks like mother board replacement time...I sent you the tag for our machine... Thanks Jason
