PowerEdge T610 Integrated RAID exception Detected

We have a PowerEdge T610 it is configured as a RAID 1, Thursday evening everything was fine, Friday Morning machine was in a constant reboot. We have a Hard Drive in slot 0 that has a green status light blinking, and a Hard Drive in slot 4 that is blinking green and amber. 

Attached is a picture of the error received during bootup. My thought is that the Drive in slot 4 is going / has gone bad? 

Our Operating System is Server 2012r2. I cannot get it to load and of course the backup tape has not been changed out in sometime, so I do not have a current backup of the system. Any help would be greatly  appreciated.

You can check hard drive LED indicator details from below link (Refer section "Hard-Drive Indicator Patterns for RAID" on page 18). Blinking green and amber looks like a predictive failure. Can you go to controller configuration utility and check status of virtual disk and physical disks?
